And I'm guessing ilevel has taken gearscore's place hasn't it?
Yeah, ilvl is the built in system they have now, so instead of adding up all the scores on your gear, it just takes the average instead. Only real difference is that ilvl is used by blizzard to determine if you can get into dungeons and gearscore was only used by players.

I can't really reply exactly anything new about your three question, but I'd have one question for you :
Why did you stop playing WoW ?
and
Why would you want to come back and what could make you come back ?
I'd say those questions might help you answer your own question as if you should start up again or not.

1.) How accessible are Arenas, rated BG's, and regular Battlegrounds? Keep in mind, I never played Cata. I remember in the later stages of BC you could at least get some mediocre starter PVP gear from honor so you didn't get destroyed in all PVE gear. Is that a similar case, or am I going to get steamrolled with a very tough path to take?
2.) How accessible are lower difficulty raids? Is it tiered like BC, or straight to the top like Wrath was? I'm not really intent on any hardcore raiding, but I have no idea how MOP gauges with newer 90's.
These are more accessible than ever thanks to new heirlooms and the ability to queue for random battlegrounds, dungeons, and raids.

So it was just the PvP gear gap that turned you off? Nothing to do with the questing, leveling, raiding, farming cobalt, the Dalaran cooking dailies, or anything like that?
I hate to break it to you, but PvP gear is even higher-level now than it was in 3.3, so you have even more catching up to do now than you did back then...

I think PhantomScourge is referring to having to play "catch-up" mid-way through the expansion with players who have been farming PvP gear from the start. Yes, you can get crafted starter PvP gear but with each patch/season, Blizzard puts out higher item-level gear that can only be earned by running Arenas & Rated Battlegrounds.
Blizzard is hoping to bridge the gap between new PvP players and veterans with some big changes coming in Patch 5.3. Here's

I originally quit playing WoW because I wanted to focus more on schoolwork and Wrath of the Lich King wasn't my cup of tea at the beginning. I came back late into that expansion, but I was pretty far behind and never really caught up. So, I just kind of quit right before Cata and had a lot of other issues going on at the time, so I put it down.
I want to come back because Mists so far looks like a fairly good expansion, I don't have any personal issues at the moment, and I don't have much else to do. And retail WoW is currently outweighing the satisfaction factors of the private servers.
I don't think you have anything to lose really.
What I suggest you is to ask for a Scroll of Resurrection on the server you were or on one you are interested to. It gives you Cataclysm for free (so you only have MoP to buy) and you also get a free level 80 on the server which the person who sent the scroll is.
It's gonna take time to catch up, but if you have the money for subs and MoP, go for it, the new content is pretty good.
